Flu shot clinic

We have a limited supply of seasonal flu shots and will have our first flu shot clinic on Tuesday, October 15th from 9am-5:30pm at Health Services (located at 3240 Luther Ave.).  If you need or want a flu shot please come by our office at that time; no appointments are needed and no appointments will be made.  Future flu shot clinics will be scheduled depending on our supply and time availability.   Please check your email for correspondence about future flu shot clinics. 

If you would like a flu shot please do the following:

1. Bring a form of payment (the cost is $20).  We take cash, checks, and most credit and debit cards (not American Express).
2. Read the VIS (Vaccine Information Statement) that was sent in an email sent out regarding the Flu Shot Clinic; this will answer your questions about the flu shot itself.
3. Print the Screening Form that was sent in an email sent out regarding the Flu Shot Clinic, fill out (in pen), and bring it with you to get your flu shot. 
4. Also, please wear clothes that allow us easy access to your upper arms.
