Flu Shot Clinic

CLU students, staff, and faculty,

We have a limited supply of seasonal flu shots and will have flu shot clinics on Thursday, October 16 from 8:30-11:00AM and Tuesday, October 21 from 1:00-4:00PM (located at 3240 Luther Ave.). If you need or want a flu shot please come by our office at that time; no appointments are needed and no appointments will be made. Future flu shot clinics will be scheduled depending on our supply and time availability. Please check your email for correspondence about future flu shot clinics. Spouses of staff and faculty are also invited to this flu shot clinic.

 

If you would like a flu shot please do the following:

1. Bring a form of payment (the cost is $20). We take cash, checks, and most credit and debit cards (not American Express).

2. Read the VIS (Vaccine Information Statement) by clicking: http://www.callutheran.edu/health_services/documents/InactivatedFluVIS2014-2015.pdf this will answer your questions about the flu shot itself.

3. Print the Screening Form by clicking: http://www.callutheran.edu/health_services/documents/FluVaccineScreeningForm14-15.pdf fill out (in pen), and bring it with you to get your flu shot.

4. Also, please wear clothes that allow us easy access to your upper arms.
