Affordable Care Act 1095-C Form

  • Cal Lutheran mailed the 2015 1095C form the week of March 7th, 2016, to employees eligible for medical insurance coverage
  • The 1095C form proves Cal Lutheran offered health insurance during the 2015 tax year
  • The 1095C form is for documentation purposes only and is not necessary to file 2015 taxes
  • The 1095B form will be sent by the medical insurance carrier the employee was covered by during the 2015 tax year
    • If you were covered by more than one medical insurance carrier during 2015 you will receive a 1095B form from each insurance provider
