Eligibility: All regular and long-term temporary Faculty and Staff with an FTE of 0.75 (30 hours per week) or greater are eligible the first day of the month following the effective date or date of hire.
Provider directories are available on the Anthem web site Anthem Blue Cross: California or in the Human Resources Office. To locate a dentist on the website, select "Dental Complete" network.
Plan Year: June 1, 2023 to May 31, 2024
Note: monthly costs are based on a 12 month period
- No cost for employee coverage
- $60.52 per month for employee plus spouse
- $75.01 per month for employee plus child(ren)
- $133.44 per month for employee plus family
