Please note: The forms below are only to be used by the Deans, Associate Deans, or Program Directors of the School of Management, School for Professional and Continuing Studies, and the Graduate Schools of Psychology and Education. If you have questions about online course development requests, please contact the appropriate dean for further information.

• Faculty is not compensated: For initial requests for online course developments being built with Digital Learning, please fill out the Online Course Development Request Form.
  
  
• Faculty is compensated: For online course developments that faculty will be financially compensated for developing, please fill out the Online Course Development Contract Request Form.
