COVID Vaccination Requirement - Process Update for Students

Dear Cal Lutheran Students and Student Workers,

This email is a follow-up to last week’s message regarding the Cal Lutheran COVID vaccination requirement deadlines, details regarding exemption forms, and how to submit documentation. Please be sure to read thoroughly, this communication contains a lot of important information.

All student employees who are working on any of our campuses should plan to submit their proof of vaccination no later than Thursday, July 15. This is the same date that all other employees will need to show proof of vaccination. Student employees must be registered for fall classes before they can submit COVID vaccination or exemption documentation. If you have recently registered, you must wait 3 days from date of registration before you can submit documentation.   

For traditional undergraduate students who are enrolled to attend class on campus this Fall, you will need to submit your proof of vaccination by August 1. Students must be registered for fall classes before they can submit COVID vaccination or exemption documentation. If you have recently registered, you must wait 3 days from date of registration before you can submit documentation.   

For students in Professionals undergrad and graduate programs, you are required to submit your proof of vaccination by August 1; further specific guidance and deadlines will come in a future communication directly from the university, your school, or your program. Students must be registered for fall classes before they can submit COVID vaccination or exemption documentation. If you have recently registered, you must wait 3 days from date of registration before you can submit documentation.   

For PLTS students, you must submit your proof of vaccination by August 11. Students must be registered for fall classes before they can submit COVID vaccination or exemption documentation. If you have recently registered, you must wait 3 days from date of registration before you can submit documentation.   

To submit your proof of vaccination:

  1. Go to the Health Services Health Forms webpage
  2. Click on the purple Student Health Portal button to sign in with your university username and password. 
  3. Once in the Student Health Portal, look in the upper left and click on “My Forms.”
  4. Click on the COVID Immunization History form and complete it.
  5. Upload a legible scan or photo of your immunization card on that form or by clicking “Document Upload.”
  6. You will receive a secure email after your record is reviewed by Health Services.  

To access the exemption forms: 

  1. Go to the Health Services Health Forms webpage.  
  2. Download either the Medical or Religious Exemption Form from that webpage and complete it in full.
  3. Go back to the Health Forms webpage and click on the purple Student Health Portal button to sign in with your university username and password.
  4. Click on “Document Upload.”
  5. Select “Exemption Form” and upload a legible scan or photo of the entire completed form.
  6. You will receive a secure email after your form is reviewed by Health Services. 

Students in all programs who enroll after the date proof of vaccination is required will need to submit proof within 5 days prior to the start of the term.

Please know that Cal Lutheran will work with students who may have difficulty accessing vaccinations. Undocumented immigrants and international students are eligible for the vaccines with a Cal Lutheran ID. If international students are unable to access vaccination prior to their arrival at Cal Lutheran, they will be expected to start the immunization process during their first week on any of our campuses. All vaccines authorized by the Food and Drug Administration (FDA) or World Health Organization (WHO) will be accepted to meet the requirement.
